You are depleted! I took 40,000iu/day at one point to get me up to a decent level.
Now I am on 10,000/day to maintain. So, don't worry about 5000.

Under 30 is chronic. Should be at least 40 and 80 is better.
Quest reports lower value than Labcorp by appx. 20% so a 28 by quest is a 22 by Labcorp.
The mixed reviews is the guy pumping the Marshall protocol says to not take D-3 but the adverse effects of not having adequate HD-25 levels are pretty well documented.

You could even take 10,000 IU's for a month, then drop down to 5000.
People with chronic lyme and TBI's should be at the high end of the reference range.

Yes, you can have a herx like reaction. I started at 1000iu tabs and ramped up over a month to 5000iu. I think it was source naturals that had the 1000iu tabs (No financial connection).
I also took them at night so if I felt bad I would hopefully sleep through it.
You could try every other day dosing until your body adjusts to it.

To be healthy, you'll need to get that up. Only cautions are:
- it could mean killing bugs, so you need to go at the speed the rest of your body can keep up with (thyroid, adrenals, detoxing, ...)
- if you have autoimmunity, it could mean more attacking of your body, so you may want to consider researching/dealing with that first

If you feel like your herxing, try a different brand. Or even D2. I did horribly on the typical d3 oil capsules. I am doing much better on dry d. I alternate between country life dry (which is d2) and a Kal d3 that is sourced from fish. I Winer about the lanolin which is where most d3 is sourced as a potential allergen for me. But I do have a ton of allergies...including animals... Do you have any allergies?
I know d3 is supposed to be the best source, unless your allergic to lanolin (even though most companies say it is so purified it is no longer an allergen). Buy I do feel much better since taking d supps, once switching. Sigh... I know I'm adding another thi g to think about and try.

I herxed & continue to herx somewhat, all depending on how much I take.
But I definitely herxed the first few times I took it and actually it was different than any of the other meds I took. With pins and needles and things feeling like they are moving in my body. End up being very tired & debilitated for the day.

FYI, vibrantnutra.com has a D3 that is vegan, completely sourced from plants, no lanolin in the process at all.
