If so, how much? Do you increase your dose during a flare or herx? I'm on 25mg full replacement dose, but sometimes I feel like this is not enough. Most endos I've seen are not very Lyme literate so I don't get much response from them.

I'm on 22.5mg daily. I keep it steady, not really having any flares/herxes these days, so don't have that issue to deal with one way or the other.

I know it's temping but, please, do not go over the amount you are currently taking.
Perhaps your other adrenal support needs some adjustment - or your energy output may be too much for what your body can do at this time?

My son takes a physiological dose of Cortef. Even though herxing is stressful, therefore requiring more cortisone. I would not recommend increasing your dose without the doctor's advise.
I knew someone who did this during herxing and during stressful days. They ended up using way too much.
It is a very individual medication. It all depends on how well your adrenals are functioning.
My son has his adrenals tested annually to see if he still needs to be on it.

I don't particularly like taking more than I have to, but it's hard for me sometimes as my adrenals do not work at all (haven't since 2006) so they do not help me out with an extra bump of cortisol here and there for stresses. If I take 25, that's all I get.
The general rule given by endos is to double or triple dose during moderate to severe illness. Problem is, what is supposed happen during chronic illness? How much cortisol is the body supposed to be producing during prolonged stress?

Your questions are adressed in the links posted above. Be sure to get the book by James Wilson. While not about lyme, it's the place to begin.
Also see the article by Connie Strasheim. It is lyme specific.
And be sure to read the thread about the damage regular doses of steroids can do for those with lyme. It's very important.
While we'll all love a support that really was the best thing since sliced bread, the reality is that during an infection such as lyme, IMO, it's impossible to really feel good.
No treatment plan, and no supplement is a magic bullet for THAT day, or even that week or month.
This takes time, persistence and the very best lyme-educated medical guidance possible to help keep our boats steady.
REST is key with adrenal damage, too. Even blocking all stimili at times is vital, even conversation, light and sound.
This technique is used medically for those in adrenal failure. That would be along with steroids but, for those with lyme, the typical dosage of steroids for those without lyme is different.
Perhaps you could ask your LLMD if they know of a LL endocrinologist with whom you could consult - even if just getting their thoughts in general from a distance. Phone consults can't be specific if you have not been seen in person.
Or maybe your LLMD could consult, though I think there is only one or two LL endocrinologists in the whole country, there may be others abroad. And ILADS doctors do stay in touch around the world. -
