Does anyone know of a doctor who will authorize an exemption from vaccines due to moral, ethical, religious or medical reasons in the NY/NJ/CT area?

The first thing I'd suggest you do is to get titers tested for all the vaccines you're concerned about.

My point is that you might not even need to address the exemption issue if you test the titers. This type of testing is typically covered by Quest in my experience.
Oftentimes, kids have acquired immunity to pertussis, diptheria, chicken pox, etc. without having had the shot.
My advice is to do the testing and take it from there. It could end up a moot issue if your child already has immunity to the shots in question.

I just found out that I will need this today so I never actually tried to get anything like this before. It's for me & I had all these vaccines many years ago. I don't want to get them again.

Good websites. Not familiar with either site.
If at the end of the day you need the shots, there are homeopathic protocols you cam follow to miimize/eliminate side effects. I recently posted about that.
I will try to find what I posted about that.

Guess it's too late now, but we were in a similar boat.
Our doctor gladly wrote a note that said that our child's immune system was too overloaded with infections to be able to handle a vaccine at this time, and that it could cause terrible harm.
Happily, the school was fine with it.

Did you ask your pediatrician? If there is no luck there, I would try to switch to a more open minded family doc - there are many out there now who would write a letter for you or sign the exemption form. The more open minded ones are often D.O.'s. And I would be shocked if a DAN doctor didn't do it.
Does your state permit a moral or religious exemption? You shouldn't need a doc for that!!
