I know - not supposed to reshield needles - but I always do.
Today injected B12 with 27g 1/2" needle. Smaller size than I usually use.
I did my usual routine after injecting- carefully holding syringe with needle in one hand - cap in the other. Got the needle in the cap and pushed down to secure it.
Then ouch!!! - what just happened?
When reshielding my aim must have been bad because the needle poked through the side of the cap. It bent when reshielding and the tip was sticking out the side/exposed. Freaky looking.
I had no idea this could happen. Geez. Now I know. Be careful shot givers!!

I've had that happen. Now I press it down on the counter top. Why aren't we supposed to put the cover back on?

Tutu - you have had the needle poke through the side of the cap?
In the past I have only pressed on a hard surface to make sure that the needle is "locked" in the cap - when off the syringe.
I had made a mistake after untwisting the needle and cap from the syringe. I thought the needle was firmly in the cap. It wasn't. So where was it? I had to do a needle in the haystack search in the area between my bed and the sharps container. Now I definitely "bang" and make sure the needle is "clicked" in so to not fall out of the cap. Yikes.
That is a stupid feeling - doing the hard part correctly - ie self-administering 2cc of Bicillin. Tada!!! Then wondering - where the heck is the needle.
