My llmd recommended methyl folate because of my inability to tolerate meds/herxing. I guess she thought methylation is off. I am already taking methyl b12. Did adding methyl folate help you?
23&me shows mthfr to be heterozygous.

No. Not with the herxing, for me personally.
Coffee enemas and IV glutathione helped me more.
But, take methylated b vits is easier to take. (I used to barf up normal b vits before and not know why...now I do).
My body "enjoys" methylated b vits.

It helped me. I was doing really well with just the methyl cycle supplements, diet and LDN. I take folinic acid instead of methyl folate because I am hetero for COMT.
I am hetero for MTHFR. My blood levels showed I was low in folate. Take it in the evening because it might make you sleepy.

I have one of the gene methylation mutations.
None of the over the counter ones did anything noticeable for me. However, got the Rx Deplin form, and it has helped considerably.

B12 and the methylfolate work together. If you have methylation issues, you can't detox properly and you can herx.
My doc ran the MTHFR gene test on me and I have one of the genes that makes it tough to detox. I have trouble taking a lot of meds and this explained a lot.
"Usually in severe cases, the symptoms are due to toxin build-up; please note the methylation defect does not cause Lyme, which is a tick-borne illness. But the infection causes ammonia, quinolinic acid, acetylaldehyde, etc… and methylation defects reduce the person’s ability from properly detoxifying, repairing the damage and fighting the infection and co-infections. This explains the severe symptoms that come and go in some people, and why some folks herx so badly and others don’t."

MTHFR is not improved with methyl-B12 alone...need to make sure to get the methyl-folate too.
May not notice a difference right away...it can take a while for the body to process through the built-up toxins, and indeed some find they need to start methylation supplements slowly or the toxin mobilization is too much to tolerate...
Some believe Folic Acid (the synthetic version used to fortify foods and in low quality B vitamin supplements) can block the ability of the methyl-folate to be used properly in the body, also, so you may need to avoid the synthetic Folic Acid also.

Nefferdun, According to 23&me I am Homozygous for COMT V158M and COMT H62H. do you know if that means I shouldn't take methyl folate?
All Mthfr are good except MTHFR C677T(Hetero).
Unfortunately I still don't have the best grasp on this methylation and genetic stuff.

It depends on your VDR status. Those with COMT variants with normal/non-variant VDR's need to be careful not to get too many methyl groups (e.g., take hydroxy-B12 instead of methyl-B12).
This is according to what I've read in Dr. Yasko's methylation protocol information.

Thanks Razzle, I am thinking about trying hydroxy b 12. I have methyl b 12 now but it doesn't seem to be working like it did. VDR Bsm is +/+ and VDR Taq is -/-. VDR Fok said Not Found for some reason.
