I am resurrecting a thread I started a few months ago.

Trying to find out levels of PenG required to be effective. How many injections of Bicillin-LA required to achieve these levels.

Granted, this is from over 50 years ago - but very much worth a read:

Particularly p255 - under Discussion;

"The amount of the dose in a given volume injected into a single muscle mass determines the absorption of the product and it is clear that the absorption is slower when the volume and the dose are high".

Also mentions some interesting things about irregularities in the levels.

Researched Neuro-S to compare to Neuro-Bb.

Edited - URL too complicated.

If interested google "Johns Hopkins ABX Guide 2012 - non-benzathine" pages 336-337.

"All neurospyhilis requires parental treatment with non-benzathine penicillin at concentrations that cross the blood brain barrier".

For months I have had nagging concerns about Bicillin-LA.

I read where low levels are effective.

Then I read where levels need to be high.

Is the .03 u/ml the low level that is effective?
That a 1.2 mu injection of B-LA appears to maintain for at least a week?

Why do folks do 2, 3 even 4 injections per week?

With benzathine in the B-LA - how are higher levels achieved with those many doses per week?

To me - something like Bicillin-CR makes more sense - as the levels or "spikes" are higher.

Crossing the BBB better? (higher levels needed for Neuro-Bb?).

I know - who am I to question the Lyme gurus.

I just wish it could be explained how they arrived at the usual 1.2 mu twice a week dose.
