Below is the report from the first mri that was done.
**spoiler alert** The neuro looked past differentials and went straight for the ms dx.
My question is this.. It indicates "chronic tissue loss". Does that mean what I think it means?
I asked the neuro at follow up, and was told that it was lesions.
----------------------------------------------- Study Performed: MR head wo/w con REPORT STATUS: Signed MRI Brain Pre and Post IV Contrast:
History: Vertigo. Visual problems. Stumbling. Numbness in arms and hands. R42 dizziness and giddiness.
Pre- and postcontrast multiplanar MRI evaluation of the brain is performed both prior to and following intravenous administration of 10 cc gadolinium based IV contrast.
There is moderate degree of nonenhancing multiple bilateral white matter T2 and FLAIR hyperintensities,
some demonstrating hypointense T1 signal of chronic tissue loss.
There is no evidence of abnormal signal or restricted diffusion. There is no abnormal intracranial enhancement.
Findings are suspicious for demyelinating disease.
Differential may include neuro- Lyme's or microangiopathic changes with chronic infarctions or possible vasculitis or migraine sequelae.
There is no extra-axial collection or mass-effect or midline shift.
The ventricles and sulci are within normal limits.
The posterior fossa and the sellar and parasellar regions are unremarkable.
Mastoids signal is normal. The flow voids at the skull base are unremarkable.
After contrast administration there is no evidence of abnormal intracranial enhancement. Impression:
1. Nonenhancing multifocal white matter lesions with foci of tissue loss suspicious for demyelinating disease
with differential of microangiopathic change and chronic infarctions and differential as above.
2. No abnormal intracranial enhancement. No abnormal signal of restricted diffusion.
