My GP has tested and treated me for Lyme's several times in the past 10 years. Each time symptoms disappear then return. Now they are becoming chronic. I can no longer sleep for longer than a few hours. My wife shares some of the same symptoms I have.
I need a reference for a LLMD in Central NJ. One that acknowledges a possibility of advanced Lyme's and takes health insurance.

Sending you a pM. But a really good LLMD is likely NOT to take insurance, it's the nature of the beast. Most (if not all) doctors who are willing to treat aggressively do not accept insurance. Good Luck to you.
