I'm looking for a LLMD in either Middlesex or New Haven counties in Connecticut, preferably one that takes insurance. Thank you.

Many LLMD's don't participate in insurances. The link will explain why.

You can request a LLMD through ILADS here;

There is a message box where you can state you are looking for a LLMD that accepts insurance but they are hard to find.

Start a medical folder with all your previous blood and urine work ups, procedures, medical doctors seen, hospital visits, a list of all your symptoms and medications and take it with you to your 1st appointment with your LLMD. Get your finances in order, Lyme is expensive to treat.
