If your LLMD is in another state, you will have to arrange everything with the local hospital and home health company of your choice.
When I had my first PICC placed the hospital did not want to accept my LLMD's order because he was out of state. Had to get my local PCP to order it. So my LLMD faxed the orders to my PCP, he signed off on the orders and faxed them to the hospital. That way everything was done as my LLMD requested.
When I got my second PICC placed they accepted the order from my LLMD. I guess it was easier that time since he cosigned everything for the first order.
The ports are surgically implanted. You will need to see a local surgeon to have this procedure scheduled. Many insurance plans require referrals. Some do not, you'll have to check on the specifics of your plan.
Make sure that the facility that you choose is a preferred provider for your insurance plan. And get prior authorizations done if necessary. It is good to have them in writing, to know specifically what they will cover.
Your insurance company will probably have a policy on treatment of Lyme disease. Find out how many weeks of IV are covered specifically. You do not want to get stuck with unexpected bills.
Hope this helps answer some of your questions. I'm sure that your LLMD has experience dealing with this too if they prescribe IV's frequently.
