If you still have symptoms, I wouldn't stop treatment for even one week.

I don't think it's necessary to stop unless you suspect the symptoms are due to medication side effects. I also doubt you'd relapse from Lyme if you took a few days off. Coinfections seem to come back quicker than Lyme.
Of course this is just an opinion, you still need to trust your doctor's wisdom and guidance.

I don't know why your LLMD would suggest you take a break when you still have symptoms and you have been tolerating your treatment.
You have not addressed co-infections. Zithromax is usually taken with another drug; it's not too effective as mono therapy. It may also increase the ear ringing.
Have you tested for co-infections? I would consider bart and/or babesia treatment.
You could try Bactrim DS and zithromax with artemisinin. The Bactrim DS would help with bart and babs. The zithro would hit the cellular form of lyme. You could also pulse flagyl or tindamax for the cysts. This might not be strong enough to cure bart or babs but it will help.

I personally wouldn't wait... I found out the hard way. Long story short I switched Doctors and the new LLMD wanted to run lab tests and I had to stop all medications for 2 weeks. Then I got my Hickman line in and had complications... so I couldn't start the IV yet... so currently I'm 3 weeks off oral antibiotics and this is the WORSE I have ever felt!! I feel like my Lyme and Bart are having a blast in there! But at least now I know the oral meds were working somewhat in keep things from getting really bad really quickly! I hope to start IV next week.

Nicole,
You might want to consider treating for parasites, a much overlooked co-infection in this disease.
Most LLMDs don't treat for this infection, unless they are trained and or follow Dr. K. guidlines.
In a recent conference, he stated that parasites are PRIMARY and Lyme SECONDARY in this disease. It's KEY in getting well.
Treating for parasites with herbs and salt/c saved my life and turned things around for me after 4 yrs of abx incl 6 mos of IV rocephin.
I had SEVERE neuro Lyme, with a bullseye being found on top of my head by LLMD. Please don't ignore this.
