There are Independent Living Centers around the country - it's a national organization. You could check online and see if you have any in your state
and do a phone consult if not close to you, about what's available in S Dakota, then see what they know about other places you might be interested in.
Otherwise, to contact ILCs where you're thinking of going, again, to have an informational phone call. You 're going to have to talk with people in the know.

i know when i was fighting lyme and raising a family and not knowing how i could survive it i looked into two short term disabilities (i am foggy so may get this mixed up some) but there was one connected to my retirement and ssdi
turned out niether worked for me
the retirement one you had to have a fatal illness (i still don't get that???? short term disability for something that will kill you
and ssdi is so complicated and takes so long to get on it didn't work
i did things like called in sick regulary so i had a shorter work week
pulsed so i killed bugs but was at my worst on days off
slept on a mat in a closet at lunch time
slept on my desk at times
i'm not saying this is a good plan...just how i survived until i was forced to retire early
