I made a very simple excel spreadsheet with enough columns for the days of the month and just filled in the numbers for the dates by hand 1,2,3 . . . ). I wrote the month at the top of the page. I printed off several sheets and put them in a folder with brads that I kept in my planner.
For the rows, I write each of my main symptoms in a cell/box. For example, one row might say, "fever," another might say, "fatigue," etc. I also made rows for "other" when those, "SURPRISE, you have a new symptom" popped up and would write it in. I also made a few rows for notes. Then, I put a check mark by the symptoms I'm experiencing that day. I would even rate each symptom 1-10 sometimes.
I have to admit I've been slacking on the recording lately, but I certainly found a flare cycle when I was better about tracking symtoms.
I tried two Lyme disease tracking apps that I didn't like, but some people may like them:
