For Ct residents, here is the link to the Dept. of Public Health complaint form, for use by patients who have been mistreated and/or misdiagnosed by their non-Lyme-literate doctors. You can print it out, complete it, have it notarized, and mail it in.
Some on this board will say you can't or shouldn't file a complaint against a doc who was only following established (IDSA) guidelines. Hmmmmmmm. To that I would respond: Where have we heard this before, about "just following orders"??
The State Medical Board could rightly claim that they have not been made aware of any huge problems with Lyme misdiagnosis based on the lack of any complaints.
For states other than CT, I hope others will post links to complaint foms/procedures for their State's Public Health dept.
