Hi everyone!

I am doing IM rocephin 2 grs/day- 5 days/2days off, I am mixing it with lidocaine 4.2 ml per 2 grs of rocephin and STILL very painful, I also get rock hard lumps on the buttocks that hurt like h......l.

Does anyone has any advise for me? What size needle do you use? How fast do you inject it?
Do you inject the full 2grs in just one spot?

TO0 many questions, sorry, any advise will be greatly appreciated.

Damn, I have no experience with IM R! However, I do know patients with do IV R should use Actigall or another gallbladder medication to help protect it during the use of IV R.

How large is the needle? I used to inject Methotrexate into my thigh.

I took one gram a day for a while, mixed with lidocaine, and never had the problems you describe. One thought is: how long is your needle in relation to body fat. Recent research has suggests that only about 10% of American women get medicine into the muscle using a one and one half inch needle. Maybe it's just getting suspended in body fat, where it will do you little good. Also, you might try injecting half and half. That's all I can think of.

I use a 23 gage syringe 3/4", so it's tiny.

David:
I am a very small person 5' and under 100 lbs, do you think 3/4" is not deep enough? This is what the LLMD prescribed. I did bicillin for 6 months about 2 years ago, maybe I got a lot of scar tissue.

I gave my daughter IM Rocephin shots for a long time, 5 days on 2 days off. Mixed with Lidocaine. That stuff is thick like Karo Syrup so you have to use a big needle.

We used what looked like a Ginormous needle, but it was really an inch and a half. I can't imagine you are getting that thick stuff through a tiny little needle

It is a very painful shot, that is why it is mixed with Lidocaine so it kind of deadens the area. Without the lidocaine it would be unbearable.

It also always left a lump, we called my daughter "Lumpy" during that time period. Just make sure and give them in the upper outside quadrant of the buttock and pull back and make sure you are not getting any blood. IT can be very dangerous if you put Lidocaine in a vein.

If you massage the area where the lump is, it will help the mixture get into the tissue. This is a shot that you will want to just put in at a regular pace, not too fast not too slow.

You just pretty much have to bite the bullet on this one, no matter what you do it is going to hurt. We tried icing the area first but it didn't really help all that much. It might help you, you could try it.

It's hard to imagine that a 3/4 inch needle would get deep into the muscle, at all. If you can get it through a twenty-three gauge needle, I'm surprised. I used a twenty-one gauge 1.5 inch. Be careful to stay in the upper, outer quadrant. If you get too close to the sciatic nerve, you can give yourself a nerve block!
