When filling out new-patient forms at a non LL doc's office - how much do you disclose?

Do you list every TBD you've had, or just the ones still being treated?

Or none at all, if they're not relevant to the new doc's practice since you have a LLMD for TBD needs?

Interested in how everyone else handles this one,

You can list whatever you want, but don't expect them to care or ask about them. If you list them, you're considered crazy..if not, well, you're still crazy if there's no explanation for your health issues. Pretty simple logic most times.

I did not reveal any of the Lyme treatment or dx when I took my daughter to a neurologist.

I did ask him about the possibility of Lyme causing her headaches as though she had not been dx'ed. As expected, he completely dismissed the idea.

I called her LLMD to make sure the meds the neurologist prescribed were ok with her abx.

I usually list them, however I do not use the word Lyme & don't list it first.

I usually start with Babesiosis, since many have no clue what that is then neuroborreliosis, bartonellosis, mycoplasmosis...

I make a sandwich of it. [lol]

I also don't bring it up, to try to avoid invoking defensiveness. I let them read what they made me fill out. It's not likely they'd miss a list like I have.

Then if they can tie whatever's wrong to my TBDs, they get to feel "smart". [Wink]

I list everything. My LLMD is very involved with my care, he will usually request to speak with my new doctor about my situation, so I disclose all for that reason.

For the record, I usually list them too. I think I'll put "neuroborreliosis" next time, too - great idea!
