If you had to take the long, slow route and were only able to tolerate one drug at a time, what would it be?

I think it also depends on what your body tolerates. Mine is clindamycin only. Can't take the others.

Our ID specialist begins with 4 months of IV rocephin, and then does a year of doxycycline. So I would choose the doxy if I had to do only one oral abx.

Most of my my recovery came from Tetracycline - wouldn't have changed that for the world.

- Liver support would still need to go along with any abx, though, especially zith, mino or biaxin as liver support can also help to protect ears and those three are the top ones that seem to stress the ear/balance system.
Tetracyclines are on the ototoxic list (see link below).
Also, if you are having trouble tolerating, it could be due to liver stress itself. Support supplements could help, you might consider using more complementary methods, too. I'm not sure any one single antibiotic is going to be effective in the long run. Rotation might be considered.
It's good to check all drugs against the ototoxicity and porphyria lists:

It depends on what you have. If you have lyme start with doxy and then switch to Ketek as it is very powerful, but expensive. Then if you have bartonella, levaquin if you can tolerate it, Bactrim DS if you can't along with artemesnin if you also have babesia.
Biaxin kills the cellular form of lyme and helps with bart.
I would not take one abx long term if I could only take one. I would switch around when I felt a plateau.

I'm trying to come up with a new plan of attack to go over with my LLMD at my next appt.
I'm not taking any antibiotics right now to treat yeast and am having a bad few days.
Not sure if its possible to have a herx while not on any antibiotics or if its a flare??
I feel like I did when I was on antibiotics.

I liked Biaxin too. Though I have to say, high dose Amoxy is a nice cheap choice. It's readily available and relatively side free.

Best: Biaxin w/Bicillin IM. Or one or the other. Amoxy w/Biaxin second choice.
