On the self claim form, it's asking about symptoms. Do I say lyme? I have all the positive tests and currently in treatment.
I know all insurance is different and chances are they know already due to my many hospital stays.

If they are asking for symptoms, just list symptoms, not diagnosis. Maybe not the whole list.

- Fatigue & pain cover a lot of territory. Vertigo or Dizziness - & weakness - if applicable. Insomnia, too. If your heart races, put that down.
Do NOT put lyme. -

I put Lyme on mine and it worked out ok for me. But I have over 10 positive Western Blots and a positive spinal tap. I suppose I am the rare exception where my insurance accepts my diagnosis. Jess.
