I was wondering if Dr. B's Treatment Guidelines have ever been or are going to be revised.
The originals are dated 2008.

I don't think so, the ILAD's 2014 Treatment Guidelines replaces Dr. B's.
From my non medical perspective the 2014 are too technical whereas the 2008's were much easier to follow / understand.

I don't know why this took me so long to think about: Dr. B has made many public presentations over the years, not just at ILADS. In some of these I've seen posted, he give what seem to be protocol-like detail. Some good slides, too, that explain so much.
I don't have all those in my file notes, yet it might be valuable to add those to this set if anyone has some. ------------
My take is that the ILADS guidelines and Dr. B's are separate, not one to replace the other but each to stand alone . . . yet each a valuable consideration.
Much of Dr. B's 2008 update (and latest) set is still very current in how to approach with combination methods, etc. Many parts of each protocol are absolutely as vital today as the day they were written.
Yet, no one protocol is going to fit even two people. These are just templates with some key guides, though. And, as time goes by, each of the ILADS conferences add to the detail to consider as well how use of certain Rx, or new approaches can better serve patients.
I think also the ILADS LLMD authors' books out add to the set and most ILADS LLMD and LL ND certainly consider the full range when seeing their patients. -

First book is very good - yet a new 2016 protocol book is due soon from this author, too.

I've not had the chance to look into this new book yet he is a very well respected and experienced ILADS member LLMD. PROTOCOLS are included here, according to the synopsis on the order page.
