I have a feeling I need to slowly work up to it?
I also am wondering if I can't take it with flagyl because it has some kind of alcohol in it?

I have not started it , but will very soon. Personally I would start with 2 drops MAX . It may be easy , I would rather start at 2 drops, go to 5 , then increase over a week or two . No point in starting out w/ a herx that might incapacitate you for 2-3 days , IMO .

THE ALCHOL IS MINISCULE, AND SHOULD NOT BE A PROBLEM imo .

Yeah, I have heard so many varying things about all of this, it's hard to decide how to go about things sometimes! My birthday is Friday and I start another arteminisin and flagyl pulse tomorrow, so I don't want to add in something else that may incapacitate me on my bday

The dose hubby took which is maybe twice the normal dose (but suggested by Dr Buhner to him personally by email) was 1 teaspoon 3 times daily. That is equal to about 150 drops 3 times daily.
I think I started hubby at 15 drops 2 times per day and quickly ramped up by 15 or 30 drops every 3 or 4 days -- took about 2 weeks to get to full dose.
But hubby had already been treating babesia aggressively for over 12 months before adding this herb.
Hubby was on flagyl while on the herb. But he always adds his herbs to hot water and lets the alcohol evaporate so it was not a problem for him.
Personally I think the sida acuta is a great babs herb, but not sure how it would work by itself. Babs treatment for most requires multiple herbs and meds.
Hubby was on artemisinin and then switched to artemix while on this herb.
Good luck and let us know how it works out.

Oh, I have never thought about adding the herbs to hot water...it doesn't take any of the effectiveness out of the tinctures?
Right now for babs I am on malarone and arteminisin, too.

boil a cup of water and pour it into the glass with tincture drops. The steam/evaporation is the alcohol

I just took first dose and started with five drops. It packs a punch- sore lymph nodes ( swollen ) and all over fluishness and fatigue . This = HERX . I guess it is a sign it is effective. I will stay low for a few weeks, then raise dose . I want to be able to function and do something other than SLEEP .
I hope this will be helpful , and would like to hear if anyone benefits .
Does sida acuta target lyme ? From what I have read it affects babesia and bart .
