Someone in our support group was complaining about this last month. Nobody in the group had experienced it or knew what it was.
I hope someone comes along with answers for you. Then I might be able to help someone else. I'm going to do a calf pain search on here and see if there is some hits.

Me. Everyday all day. It's been constant for 2 years. Only solution that helps(besides treatment) that I've found is REALLY tight wraps. I use wraps like the ones that football players wear. And small Under Armour pants.
I wouldn't say it's my worst symptom, cause there's a million bad ones, but this one is up there. Now my treatment is working and it's slowly going away and I've lost the cane.
I poured the packet into my palm and touched my tongue to the powder, letting it dissolve, and then repeating until I finished it.
I wasn't sure what result I'd get when I first tried it, and I was hoping for relief from fatigue. But what happened was, a couple hours after that first dose, the aching pain in my calves improved.
I worked it out that if I took a packet of the B12 every eight hours, I could stop the leg pain. I did this for about two months, and then went down to twice per day.
B12 is known to help stop neuropathy, or nerve damage. So the deep calf pain was due to the nerves?
The muscle twitching is probably due to low magnesium.
What meds and supplements are you on.

I have had twitching calf (left only) which was my first symptom 8 years ago and burning...stabbing...electrical current pain in my L calf consistantly now for over 8 years. I have tried everything to help this but nothing does. They are not weak but twitch 24/7.
Been treated for lyme...bart...babs. I go through periods of time when it isn't as bad but it never ever stops...I fully believe it is permanent nerve damage from not being treated for 3 years when it all started.

If you're having twitching muscles, then you'll need more magnesium. You need Vitamin D3 to help you absorb the mag, and Boron to help your body retain it.
When you buy magnesium, look at the list of ingredients. Look for magnesium glucinate, aspartate, citrate, or one of the "-ates." Magnesium oxide is cheap, so the manufacturers use it, but it is poorly absorbed.
You'll need to take several doses throughout the day, and continue for a long time.
Try a good sublingual B12, methylcobolamine. It's not expensive, and in my case, I got benefits I wasn't expecting.

I have had this. I used to have a constant dull ache or cramping in my calfs as well as some twitching off and on.
Magnesium and various other supplements didn't resolve the issue. That was the first thing I thought of but was not the problem for me.
After looking into it more I read in multiple places that calf pain/cramps etc is a hallmark symptom of bartonella. I have never tested positive for bartonella but I do suspect that I have it based on other symptoms.
For me the calf issues have mostly resolved over the 4 years that I have been in treatment for Lyme etc. So it looks like my issues were/are definitely related to lyme and company (possibly bartonella).
Hope this helps.
