My first impression upon looking at the photo close ups is that this could harbor some very serious microbes and cause serious infections. There appears no way that can be kept clean inside.

I would not chance it. Yet, if you get one and can inspect it, or otherwise read about how it sterilize it each morning and night . . . that's what needs to be determined. Then, the kind of plastic that has contact with the nostrils. See if they have taken care with the kind of plastic used - it is a petroleum putty of sorts.

No matter how soft, it could damage the tender tissue inside the nostrils and create ulcers that would be more open to infection.

This would be the lesser concern. The hygiene concern is the most important - the chances that this could harbor infectious microbes / mold.

But, really, is the problem that the nostrils collapse? Is that what causes the problem? Never heard of that. Usually it's more internal issue or a neurological / lung / position one.

Inflammation in the sinuses is not likely to be addressed by this devise. There are other ways to approach that, though.
