I'm pretty convinced that I have Bartonella (negative serology but very positive symptom list and 4 years failure on Lyme treatment). My blood is right now being checked by Fry Labs (blood smear) and when the result will be back I will have to tell my Lyme doc what I want to take (even if negative I'll treat it).
The treatment of Bartonella is practically unknown here in Germany but my doc is very willing and I think/hope/pray she will prescribe what I want.
But what do I want? For your information: my weight is currently 115 lbs.
I would like to be able to suggest her two options:
1. Levaquin with Minocyclin and a PPI.
Which dosage of Levaquin do you take? With Mino I was previously able to go up to 150 mg a day. When do I take the PPI - before the Levaquin or after? How long before or after? Do I take the PPI once a day or more often?
I'm scared of this combo - so if I cannot support it the other option would be
2. Zithromax and Rifampin.
Or is Rifabutin better? Do I need a PPI with this combo, too? What dosages do you take? For Zithromax there are so many different ways to take it. Do you take it every day or do you take some days off? I'm planning to ramp up the Rifampin slowly - how much are you taking?

My son takes the combo of Rifampin (1 capsule 300 mgs. 2 x's day) and Doryx [a gentler form of Doxycycline] (2 tablets 100 mgs each 2 x's day) to treat Bart. He also continues to take Plaquenil and Tinidazole as well as probiotics and supplements.

Hi Gabrielle, At 110 lbs, I took 600 mg of Rifampin once daily and 100 mg. doxy, 2x per day.I did this for 5 days per week. On weekends I took 500 mg azithromycin and continued my doxy. I started at 300 mg of rifampin for the first week and then started 600 after one week. Good luck Kris

My daughter's first protocol was to treat bart (her LLMD is the lab guy). She started with azithromyacin (had to work up to 500mg daily).
After six months, added Rifampin (could only manage 300mg daily, and then, only four days a week).
At a year, he had done a lot of research on Vitamin D therapy. That seemed to make a big difference, then Babesia popped out, and we had to deal with that.
She is now on just 200mg minocycline daily (had to work up slooooooooooowly to that level), and 6000IU of vitamin D.
Hope this helps. I think zith and rifampin are the most common right now.
Start taking that D3!! You'll herx at first!

I'd strongly recommend Rifampin be in there somewhere along with some other IV drugs ( Rocephin then Doxy then Claforin)I think it really helped me with two strains of Bart. I had to stay on the Rifampin I think after the PICC came out. I could tell if I forgot to take it the next day (took it at night).

At 115 lbs, I alternate days between 500mg zithro and 600mg Rifampin.
I started on these dosages, went higher and it was too much so I think this is a good dosage to make your way up, too.
Seems to be pretty much the same amongst most of the Bart members on Lymenet.
