Please PM me or email [email protected] if you would like a generic form (that you can customize with your docs' names etc) that I have made for inviting doctors to attend, plus the press release.

I would post it here but it has doctor's name etc.

Again, the meeting is on Monday April 28th - 12-1:30 PM and includes lunch and Dr. R from NYC speaking on the myths of Lyme - demythifying them - plus explaining Lyme 101.
