They can last anywhere from a few hours to days, to some people think months. Everyone is different. Sorry there is no good answer.
Just know that it should pass. I take it this is your first herx?
What are you experiencing. and how long has it been going on?

Everyone is different. My first one was only a few days. It was the one that occurred when I got up to the higher levels of abx my LLD prescribed that has been bad, it goes on and on. Some of these will last months. There are things that help. Talk to your LLD and tell them what's going on. Don't let it get out of control.

Sometimes mine last a few days, sometimes months. Then I will have a day or a couple of hours where I feel better than I have in many years, so I know I am making some progress.
Some people herx continually for years, and then suddenly they are in remission, some people herx a long time, then get a break, then herx a long time again. Some NEVER herx, and yet still get better too.
Everyone is different.
You also have to take into account that Lyme symptoms flare with or without treatment/herx. So you can have flares and herxes both.
If you just started treatment and are feeling new or increased symptoms then you are likely herxing, and if it is too much to handle, you can discuss options with your LLMD to cut back on your meds for a bit and then go back to them, or lower your dose for a while.
A herx is sometimes unavoidable, but you shouldn't push through a very bad herx - it is basicaly subjecting your body to the damage of Lyme to a greater degree, which does not come without harm.
Discuss with your LLMD what options are available to you to work through herxes to keep them bearable.
Also, someone else mentioned doing other things to help you through the herx. You should definitely look into detoxing and organ support options. I personally make an herbal tea with sarsaparilla and other herbs that help to bind and remove the Lyme toxins.
You also want to support your liver etc. These things can help your body deal with the toxins better, and so sometimes give you lighter symptoms of herxing, and hopefully protect your body from some of the damage.

I go through little 1 and 2 day herxes, my monthly herxes last 6-8days most of the time.
If I feel I am about to make myself incapacitated, I will back off, detox, and will usually feel great in a day or 2.
