All though it has good penetration deep tissue .From what I have read.

Radioactivity was widely distributed in the rat after oral and intravenous administration, although
levels in the central nervous system were low, indicating poor passage through the blood-brain barrier.
Levels of radioactivity in tissue decreased in parallel to plasma levels with almost complete
elimination of radioactivity by 24 hours after dosing.

Good question. Most macrolides do not have good BBB penetration, being this is a close cousin, it will probably be similar.

I saw an interesting post some time ago that stated that Zithromax accumulates in high levels in the brain but has only low level CSF concentrations.

Ketek is listed as not going into to CNS. Zithromax has a poor rating for CSF but good rating for the brain, strange. Mathias is right about that.

From the group of macrolides only Rulid gets throught the BBB right into the brain.

Ketek is a close relative to macrolides and doesn't go into the brain.
