theres so much info on here about rifampin and as clarissa posted, a lot of it contradicts one another.
ive been on it for a little over a week now @ 300mg day, had to take one day off, and am now wondering if i should bump my dose to the full 600/day because im not having as severe a herx as i did when i first started it.
my gut tells me to wait a few more days and then to bump it.
also, it is paired w/ bicillin as i could not tolerate mino or zith before i started it.
i am also on zhang, plaq, and a bunch of other stuff.
is this acceptable? (like anyone is going to giveme the same answer)
i believe my dr knows what hes doing so i think itll be fine, i just dont want rifampin to become ineffective because its one of the last drugs (maybe 4 or 5 left) i have to try to smack down blo.

My approach is to always listen to my gut.....If your intuition is telling you to wait a bit more to ramp up, that is what I would do.

First - always follow your gut, always.
Secondly- I see 2 LLMD's the ILADS LLMD will not prescribe ANYTHING with Rifampin, claiming it can destroy your liver.
My non-ilads LLMd says something must be prescribed with it and he usually prescribes Septra.
But neither ever prescribe more than 2 abx at the same time under any circumstances.

I just started Rifampin 600mg with Doxy 400mg on Monday. First 48 hours were a bit rough on my gut but I think I'm over the hump.
I try to take both with food. I will probably do doxy for two months with it and then switch to azithromyacin.

I am on IV Rifampin paired with IV Rocephin, I started 2gms Rocephin daily and then added Rifampin at 150mg/day and built gradullly to 600mg daily
I have my LFT's checked ever other week
You need to keep an eye on your SHBG (sex Hormone Binding Globulin) with Rifampin so my LLMD did a pre and post meds reading as in some patients it will change this dramatically
My LLMD insists it is taken with another AB to stop drug resistance

never heard that, but unfortunately my libido is already reduced to almost nothing so i dont think its a concern, or is it?
thanks for info rianna, i am glad someone else also had to ramp up. i started on 300mg day/am and i think after 2 weeks i will be able to bump to 600 daily.

If a person is getting their proper blood work every month, there shouldn't be a risk of Rifampin doing any damage to the liver. If the serum shows exceedingly high levels at the end of the month, the doc merely backs you down or stops, until the liver ranges are back in check.
The only way I could see it "damaging" your liver, is if your doctor ignored your blood tests and kept you on it, or if you weren't having your liver enzyme levels monitored regularly.

Rifampin can esculate the SHBG, SHBG binds to sex hormones mainly estrogen and testosterone and if it rises it will make you feel very tired. In women it will make you feel like you are menopausal - elevated SHBG values can cause symptoms of osteoporosis, osteopenia, muscle weakness, poor memory, depression, low ibido etc.
My LLMD did a presentation a while back and stated the SHBG test and LFT must be monitored whilst on Rifampin.

My ILADS LLMD said that you should not miss any doses of Rifampin or you could hae an allergic reaction where something happens to your platelets. He said it was very serious. I didnt get into more detail than that- sorry

lets say he does monitor shbg upon my request, what does one do if its out of range?
i do feel tired since starting it, but that is most likely a herx
as far as the lft's and your liver dying, i dont think so..
i get a cbc and lft every 4 weeks, its never been off and if it is im sure ill be the first to ehar about it.
but i am interested in hearing what u would do in the event the shbg is off.

Derek's post is from 2008, I upped the thread because I'd like to know more about SHBG and Rifampin..
I believe you take it on an empty stomach because it won't be absorbed properly if not.
