Now seems to require prior authorization. They now require from the doctor:

1. the dose
2. how often it is adminstered
3. the total quantity requested, the diagnosis or indication, whether used as treatment or prevention, if the condition is chronic or acute, whether the patient is immunocompomised and HIV status.

To all of that, I say it's none of their business.

We had the same problem and we never got it approved. Had to switch to a diffeent med.

i had a change too...jan. 1. not sure how it will affect me yet.

seems like they are purging themselves of meds that work and pushing the ones that kill you slowly

We have Express Scripts and they paid for both of us to take Azithromycin. Must have something to do with the type of policy??

They may have partnered with Medco- a bunch of pigs if you ask me!

They are only authorizing 6 tabs for me at a time. UNbelievable. Makes no sense at all. What purpose would that serve!! Just another of their fishy tricks. [Frown]

I just got a prescription for 600 mg for 30 days filled at Costco. The generic is $10 copay. No idea what the hold up would be.
