Now seems to require prior authorization. They now require from the doctor:
1. the dose 2. how often it is adminstered 3. the total quantity requested, the diagnosis or indication, whether used as treatment or prevention, if the condition is chronic or acute, whether the patient is immunocompomised and HIV status.
To all of that, I say it's none of their business.

We had the same problem and we never got it approved. Had to switch to a diffeent med.

Just on Azithromycin? Why?

i had a change too...jan. 1. not sure how it will affect me yet.
seems like they are purging themselves of meds that work and pushing the ones that kill you slowly
