I have been diagnosed with Lyme for 4 1/2 years, taking Biaxin and Doxy for most of that time. I took about 6 weeks off of the abx because of a dental issue and the dentist had me taking some other abx for that time. I started back on the Biaxin a couple of days ago and for that time I have had really bad all-over pain. It feels like muscle and joint pain. It's almost unbearable. I have never experienced it in all my almost 5 years. Is it just a herx? I've never really had a bad herx before. Really didn't tell a lot of difference when I started on abx in the beginning. I am a little concerned about this pain. Any ideas?

I'm sorry for what you're going through. It sounds like a relapse, based on exactly the same thing that I went through. Was being treated for weeks, couldn't get anymore rx from any Dr. and developed joint and muscle pain rather quickly, that I never had before. This is what I believe has happened to you. If you haven't already, you need to get back on meds.

Since you started back on the meds and developed symptoms immediately after that I would think you are having symptoms related to die-off. I would consider taking anti-inflammatory herbs like curcumin or boswellia and possibly enteric coated proteolytic enzymes like serraflazyme between meals.
It should get better after your body has had a chance to clear out the toxins. Look at this detox thread for suggestions on how to lessen the reaction:
Are you seeing an ILADS LLMD? I would contact them and let them know what has happened.
Borrelia is slow growing. I'm not a doctor but I would think if you were on an effective treatment for 4 1/2 years you wouldn't relapse so quickly unless it were a co-infection. Have you been tested or diagnosed clinically or treated for co-infections?
IM ceftriaxone worked miracles for my severe overall body pain. I'd had pain for a long time so it took 6 months but I got some amazing relief from it. Brought the pain down by 70%.
In my opinion, your best option is to keep detailed notes of what has happened recently and from now until you see an ILADS LLMD who can evaluate your situation.

Thanks for info. I am back on meds, although slowly. I was afraid full dose all at once would really get me! Yes, I have tested positive for Bartonlla. The doctor I see is a LLMD. I got referral from The Lyme Disease Association in 2005. He did not change meds when the Bartonella test came back positive. I guess I should just give it some time but pain is crazy! I will probably take the advice to let my dr. know. I was thinking of that already and back to documenting symptoms. It has been a long time since I've done that. I see him on Jan. 24. Also thought about beefing up my detox.

IM is Intramuscular injection. Ceftriaxone is generic rocephin.
I don't know what type of bartonella you have but I'm pretty sure most LLMD's treat bart for lyme patients with more than just doxy. Levaquin got rid of my bart rash. I got the rash when I started to take doxy. Since the rash is caused by blood vessel growth I believe that doxy somehow stimulated the organism to grow, probably because it felt threatened. Check for long red/purple streaks just in case there is a visible sign that bart is flaring. Be sure to check behind knees, inside thighs and other places that you would not normally see.
I was on doxy for months before starting bart treatment and it did not get rid of the rash so I would say in my case it was not effective for my strain of bartonella.
I would want treatment with something besides doxy for bart. I might also ask for another bart test. That said, my test was negative but I clearly had bart. If it were me, I'd be highly suspicious that this flare is related to bart and if so, it seems clear that current treatment is not working and you need something else.

I am familiar with a typical bart rash. I have never had it. I may ask to get retested for bart. I did actually know what IM stands for, just wasn't thinking. You'd think I wouldn't forget - when I was in the hospital a few years ago, I was supposed to get an injection IM but the nurses screwed up and put it in my IV and I had a bad reaction. Everybody scurried around and for the rest of my stay I had to wear a heart monitor. Only one of several medication mistakes made while there. A little scary. Thanks for the info
