Hello all, My wife called Dr.S's office this morning. They told her that she has to send her history, symptoms and mailing address in one page or less to their office. Can you please give us some idea on how can we include everything in one page. Thank you.

That�s a tough one, all I can say is try your best. List all her symptoms & history, but shorten each one Dr. S. will figure it out.
You can also make the font smaller, and no bold letters.
Do not go over the one page limit, they will more than likely ignore the note if you do.
You can do it, when you have to.

Sometimes it is easier to put it down, then prune until it fits. Writing long is easier than writing concisely, for sure!
One way to organize her history is by year, one or two lines for each year. When they ask for symptoms, it is always hard to do because the symptoms vary. So, what might work is a paragraph with all the symptoms she has ever had, with the current ones underlined (make a note that is what you are doing).

Make a list of symptoms:
Date of Onset
1.. 2. 3. 4. 5. 6. 7. 8. 9 10 List any hospitalizations cause and date
Keep Everything To the Point without a lot of detail
Example: Got sick after car accident and never recovered
You don't have to write big paragraphs for them to get the point.
It's similar to the medical questionairre that you fill out at the office.

Thank you all. My wife took the effort and sent the fax.
When she first called the doctor's office, she was said she has to send the fax and doctor will go through the document if he is not willing to cure her then he will recommend one.
My wife sent the fax on 12/10/2010 and did not get any response from the office so she called the office. The people at his office had no clue if the doctor read my wife's document or not, but they told her to go to some other doctor.
If the doctor is not interested to read the patient's document then why on earth they have to send it?

Is this the famous Dr. S from MD? If so, they did the same to me. No response and after I followed up told me I was rejected and go find a different LLMD.

If you are in the DC metro area there are many other LLMD's to choose from. Try posting again in Seeking, or if you already did that look at other names.
You can also ask for input from patients of different LLMD's. People can then send you PM's, keeping the info private, since one person's experience isn't necessarily indicative of what happens with everyone.
However, if you begin to see a trend you do not like, cross that name off your list and move on to others.
