I've been infected for somewhere between 5-9 months, finally testing positive for Babesia Duncani as well as Microti which explains why I haven't improved at all from lyme treatment.
How long to see improvement? I'm on Mepron + Azith.
I had a positive FISH from Igenex as well as 1:160 IgM titer for Babesia Duncani. Any input from those of you who have actually tested positive for Babesia and treated with this combo would be of great help.

I am on this combo now, along with penicillin and fluconazole, also lo-dose naltrexone. I will say that within a few days or weeks I started to herx from the Mepron. It was like having the flu but without an obvious fever. Then I had night sweats for the first time. Fatigue returned, so did my arrhythmia, itching, and some leg cramping.
It would have been easy to think I was relapsing, but I know it was a herx.
7 mos. after starting Mepron, the major herxes are over. I am stronger and am going to the gym once or twice a week for a good workout.
Mepron kills bugs dead. I've made good progress on this. It kills babesia, but it also apparently goes after cyst and intracellular forms of Lyme.

I have been on it for almost 6 weeks. The fourth week was the worst for me. It takes that long for mepron to build up in your blood.
I was very depressed from it and extremely tired - walking up a flight of stairs left me breathless.
Then I began to feel anxious and nervous with irritability. I am coming out of the worst of it - I hope. I am not as tired as I was.
I am not so depressed or anxious although I have had my ups and downs with that. The hot flashing is not as bad.
I also felt like I had the flu when I started it. In fact, I actually thought I had the flu.

I've been on Mepron/zith/tindamax for over a year. I never herxed much, other than a few, light, fu-like days.
The key with Mepron is to work up to a dose of 2 tsp twice a day. There is a lot said that it may not be effective until reaching that dose. That said, start out for the first months at 1 tsp, twice a day, and work up when you stop herxing or per your doctors orders.
I've had major improvement from Mepron, and didn't even notice the subtle differences, until realizing that so many symptoms had resolved.

Thanks for the input everyone. And nspiker, a year of Mepron? That's a very long time. Did you ever test positive for Babesia?

I took Mepron (or for a couple months Malarone) for 20 months. It took 24 days before I herxed ...... I started a bunch of meds all at once when I started treatment and Mepron was one of them.

Every anti-parasitic medication I've taken works, so I must have something "like" babesia.

I never herxed. Just felt better, more energy, etc. I was on for about 6-7 weeks, now just had a 1-week break because I was having a reaction to another treatment that put my body on overload. Planning to re-start any day now.
I think you will see major improvement with it. Good luck!

I've been increasingly fatigued ever since I started Mepron. It's only been 2 weeks but I don't feel any improvement at all. When should I feel some improvement?
I'm trying to be patient but I've been in treatment awhile with literally no improvement whatsoever. Really hoping I feel something with this Mepron soon.
