I have...My insurance is BCBS of IL..our plan is actually a self-insured plan from my company which is managed via BCBS of IL.
I used to hate my insurance before I was Dx'd for lyme. But...now that I have a chronic illness, I have to say they have been great. As long as I pay my deductible, they rarely reject anything.
The only thing they refuse to pay is my CD-57 test which is about $50.
I have heard BCBS of Mass flat our refuses to pay for SPECT scans. I don't get how insurance companies can deny treatment when the doctor says it is necessary.

I have BCBS of MA. They covered Spect Scan as far as i know. I have not recieved a bill and it was done in February.

My child's spect was covered - needed some sort of pre-authorization - by UHC. My son had neurological issues from the office exam, so that was probably a justification.

Mine showed -3 & -4 standard deviations of perfusion (blood flow) to the frontal and temporal lobes. Quite bad and most likely responsible for the cognitive issues I combat constantly. Trying heavy anti-oxidant therapy to see if it helps... nothing so far. Awesome.
