I was bitten by a nympal deer tick just over 2 weeks ago in Sweden and now developed a 2 inch rash. Two days ago I started taking Doxy 500m mg a daily but I am not sure whether I should instead switch to Amoxy and in what dosage (1000mg or 1500 mg x 3 times a day). Also I am a bit confused about co-infections, do I need to take some addtional abx for coinfections now or if not now,m then when?
I would also be grateful if someome could recommend an LLMD for me, anywhere in the US. I would then try to arrange to have some consultations by phone. Unfortunately the doctors here in Germany are very ignorant about the LD. Just to give you an idea - my general practicioner prescrbed Doxy 200mg for 3 weeks which is totally inadequate.

You can take 200 mg twice a day for now and find another prescription ... so you make it over the first weeks.

You need not go to the US. There is an Ilads lyme clinic in Augsberg called borreliosis centrum. Look it up. They have very good labs and will do a full co infection work up, and I.v antibiotics if needed.

Since it's a recent bite, I would treat with antibiotics.
However, after almost two years on antibiotics, I flew to Pforzheim, Germany for treatment with the Bionic 880 and got well from that treatment. I spent three weeks there and continued the treatment once I got home.
But, as I said, I'd treat a new bite with abx. I'd stay on the doxy, it's better for a new bite than amoxy because it covers more infections (like erlichia). You can find treatment guidelines at www.ilads.org

I would go to dr. W in Pforzheim too and let myself get a Bionic session. You probably don't need to stay there longer as you're newly infected.
Antibiotics were not the solution for me, nor for my daughter. The bullseye rash faded for me with doxy, but the disease started in revenge later. I was undertreated with doxy-only, like you, 2 weeks.
My daughter was treated with amoxy, that did nothing for her, she got even worse, after 5 weeks we stopped it and changed to alternative treatments.
If I never took abx for borrelia, I would do like you, try 1 or 2 months on doxy, if my body could tolerate that. Amoxy doesn't catch coinfections, so it wouldn't be my choice if single-bullet.
There are loads of alternative and conventional treatment in Deutschland. You got to look around. No need to travel and lose your economies in the USA.
Extremely expensive doctors there, and you need still to pay the tickets, hotels, then keep going on and back.
Germany is much more affordable and there are good doctors. If you keep posting in German forums, you'll find your addresses to the type of treatment you are looking for.
I was treated by the INK people, the neurobioly institute from dr. K. It was good, but today I would try dr. W. in Pforzheim first (because I think it is more fast and efficient, specially for newly infected people like you).
If you are lucky, 2 months of doxy could help knock it down.
I live in Switzerland, by the way.
