I've had this. For me it was Bart!!
Irritates the CNS to a point that you simply can't sleep (cortisol stays spiked)
Many of your other symptoms indicate Bart too

My advice would be to get a sleep study and also see a Naturopathic or possibly an LLMD to have your saliva cortisol checked through diagnos-techs.
You might also consider that you may have a methylation problem and need the active form of folic acid. There is a product called folapro. Everyone in my family sleeps better while on it.
I know how awful it can be to sleep so little. For years I woke up every hour or 2 and also felt like I was going to die in my sleep. For me it was sleep apnea and the methylation issue.
Good luck, I hope you get it figured out.
