I've been on several rounds of this med and each time, my LLMD had me on a different dosage. I think you really need to call your LLMD on this one and ask him/her what you should be doing.

I tried to pm you back but your mailbox is full. The pills come packaged 24 to the container. You are supposed to take 4 pills, then 8 hours later 4 more. After this you take four every 12 hours until finished (3 days total). This is the standard way of dosing according to package instructions. I just took it every 12 hours the whole time.
Don't be afraid of it. It will probably give you a big boost.

It is a tough med but you'll be fine. Just remember that it keeps working for a few days after you are done taking it.
I got good results but my doc at the time didn't follow it up with anything and I relapsed rather quickly.

He has me taking one pill per day for 4 days to start. That seems low, but boy is my heart palpitating. I wonder if this is normal ?

I took coartem like nefferdun did.
Easy way to remember the dosing: first day I took 4 pills at 9am, 4pills at 5pm second and third days I took 4 pills at 9pm, 4pills at 9pm
I did not have any heart symptoms while on coartem. It only made me feel better.
Please, follow your doctor's advice. Maybe you are starting out with the one pill now and will work up to the regular 4 pill schedule eventually?
Do let your doctor know about the heart palpitations, OK.

Are you still on Zith? Zith and Ocartem are a huge no no? They both impact theQT interval.
You are supposed to stop all macrolides and quinolones 5 days before doing Coartem and restart no sooner than 4-5 days after stopping.
And yes the "standard" dose is 24 pills over 3 days. 4 pills on the AM of day 1, 4 more 8 hours later. Then 4 ever 12 hours after that.

Thanks Sammy, I go to my Integrated LLMD tomorrow. She did not prescribe the coartem for me. My other LLMD did. I will discuss this with her and have a glut push as well.
I stopped Mepron, Zith and Plaquenil for 4 days and started Coartem this AM. I am very tired. Hope I get some relief soon.

Coartem did give me heart palpitations and I was only taking 2 pills twice a day for 7 days.
It got so bad by the 5th day that I went to my primary and he did an EKG that came back slightly abnormal.
So I stopped at that point. And I still got the benefits from having taken it.
Were the palpitations a herx? Possibly but if they continue I wouldn't ignore them.
Mine stopped after being off it for 3-4 days.
I later took another course of it and that time I took magnesium glycinate with it and the palpitations were not as bad.

I wonder why there are different dosages. I vaguely remember a famous lyme doc at the ILADS conference in Oct (ILADS stream), say that you are supposed to take it 3 or 4x a day for 3 days. My doc said different. He said 2 x a day for 5 days (I don't know if this is a new thing).

Is there any benefit to taking glandulars if they give you a wired feeling? It feels like it's too much coffee and recovery time is needed when it wears off. Feels like it's too much, but is it doing some good in the long run?
Is the action of the glandulars to provide what the adrenals normally do and give them a rest?
