Does anyone have a success story about appealing an insurance denial of IV abx (especially in NJ)? I would love to know what worked for you. Stories about Horizon particularly welcome.
Did anyone need an attorney and if so please share your experience and attorney's name?

There were some good threads on insurance appeals, but they have mostly been deleted, for some reason.
Here is the only one I can still find, don't know if it is exactly what you are looking for.
There is someone in NJ who is very knowledgeable about this subject, maybe she will chime in.
