Definitely take with food!!! Alinia can cause nausea otherwise. My LLMD has me on one 500mg pill, twice daily.
Take plenty of probiotics, separated from Alinia or other antibiotics/antiparasites by at least 2 hours. And get the good probiotics. You will not regret it.
Also, I had a lot of gas (and the accompanying stomach pain) the first week or so, so stocking up on GasX in advance might be a good idea.
I thought it was totally going to kick my butt (and my gut!) for a while, but after two weeks, the major side effects have settled down.
Oh, and for the first week, Alinia caused extreme sleepiness. Like when you take one too many Benadryl. So if you're taking anything to help you sleep, or any opioid pain meds, monitor yourself and be sensible if you notice things are getting too intense.
I, for example, had to cut my dose of Klonopin in half after a couple of days of 20-hour sleep and four hours of "wakefulness" when I had to fight to keep my eyes open.
Again, that side effect has pretty much disappeared now that I'm over two weeks in.
So to make it short and sweet: Take with food. Take probiotics. Have Pepto or GasX on hand. Don't drive or use heavy machinery until you know if it's going to knock you out for a couple of days.

I agree with Anna, take lots of probiotics. The GasX is a good idea.

Dr. K says you should take a double dose. so 1000mg 2x / day. Do this for 20 days straight and then take it 2x / week for several months.
