Is Rifampin and IV Rocephin enough to see improvements in Bart? I think my Lyme is improving, but Bart is not. In the past I was on Rifampin/Zithro or with Doxy as well and saw inprovement quickly (few weeks).

I think you can build a tolerance by going on and off Rifampin - it happened to me. JMO.
