Melatonin (1.5mg sublingual) and a strong tea of "Organic Nighty Night" by Traditional Medicinals, scoop of CALM-Calcium and Magnesium, and 7mg Boron.
Sometimes 25 mg Bendryl also if I'm feeling restless.
I have a hypnotherapy CD, a fan for the white noise.
Also Sleep Rain, Sleepy Ocean on my Ipod if needed.
I swear, getting ready for bed is quite the ordeal. All in all, the boron seems to have made the biggest difference for me falling asleep.
It's a new addition to my regimen.
And when all is said and done, I still wake up about 6 times between 2 am and 4am but I think that's Babs.

I tried Zolpidem, Ambien, Melatonin, L-Theanine, Diazepam, Savella, Lorazepam, Trazodone and magnesium. The only thing that worked was Lunesta which gave me 4 hours.

I went through 3 years of sleep hell. One thing that has helped me tremendously is supplementing with the amino acid Tryptophan. It's a precursor of melatonin and serotonin. It has a calming effect and supports deep sleep. I've been using Doctor's Best brand. They use Ajinomoto's TryptoPure - it's pharmaceutical grade. I have never slept so deep in all my life. I also take 500mg of GABA (gamma aminobutyric acid) which is referred to as the 'brain's natural calming agent' as well as Magnesium. I take all three supplements at bedtime

Thought I would through a couple of my thoughts here as well just in case any of it helps. You might be doing much of this already.
I went through a couple of years of sleep hell as well.
One of the best sleep aids I used was 'Power to Sleep PM' by Irwin, a combination of Melatonin, Gaba and Valerian. Combining that with the CALM magnesium before bed really helped
I cut most of the caffeine out of my diet (I am sure you already have just putting that out there) and that really helps
I take 0.5 mg of Ropinorol (requip) for restless leg (I am not completely convinced it is RLS as it is more of leg and arm tremors in the evening, thinking it might also be a lyme symptom that is just hanging on) - I consider that one drug and the relatively low dose that I take as an almost 'miracle drug' - I cannot sleep properly with out it.
I still have issues with sleeping during herx times, but the ropinorol really helps.
