So she hasn't been feeling very well lately so I told her to ask her doctor to test her CD-57. It was 38. Her doctor said he's never even heard of this test and he doesn't know what he can do for her now.
What do you guys think, what else can cause it to be low?

chlamydia pneumonia is documented to have caused low CD57 in some instances
some docs believe mold with mold illness can do it too, although I have not heard that there was a well-documented cases to prove this has happened
HIV has also been documented to do it in some instances
An ILADS meeting maybe last October or something like that had a good presentation on this test, which is where I am getting this from. I bought the presentation on audio/internet for something like $20 from the ilads website, and am going from memory here, having heard it almost a year ago now. It might still be there. There is also a thread that someone posted on this presentation, someone who was actually there at the talk, you might find if you search.

I think several conditions can lower CD57, which is why some drs feel it is not all that useful to measure. There is a good article online by Ginger Savely NP describing her beliefs on how the CD57 is useful. Maybe that article might be helpful to your or her dr.
Different labs do the test differently so whichever lab you start with stick with it for comparison of progress, otherwise you are comparing apples and oranges. For example, my original CD57 through Quest was a 2, yet through LabCorp was a 48. LabCorp at that time ran the test in accordance with the Shoemaker profile recommendations if it was sent a specific LabCorp office, Jewish National I think(?). I have been out of the loop on all message boards for some time now, but have heard all that is changed now, but not sure if that is true or how it has changed.
My personal opinion is it at least the CD57 is a measurement that can confirm something is seriously going on. Your sister's count of 38 is very low.
In the last 5 yrs of treating and testing it regularly mine has come up to around 150 (up from 48), though not tested or a year...... but, I remain remain quite ill and bedridden. So, an improved CD57 is not a directly correlation to feeling better, though I have to say without a doubt that seeing the progres of CD57 coming up thas kept me engaged with treating. i think without that i would have totally given up all hope. Some drs think it means I may be close to a breakthrough of change in symptoms. I can only live in hope.
