Oh my!
I get it I totally do.
Had RMSF lyme and still bart.
Rifampin herx was a nightmare 10 days. Then got better.
Levaquin and rifampin woulda sent me to the nuthouse.
Hang in there. Let a close trusted friend know you may need a daily call or visit to make sure you are OK.

I'm somewhere around 2nd to 3rd week on the combo. I had to miss a few doses to take care of my family for a bit, now it looks like I'll have to miss a few due to work emergencies.
Sucks being the boss sometimes.

Be careful with levaquin. If you have ANY tendon pain (or other pain), especially in the Achilles it can become permanent.
I'm NOT exaggerating.
Stop taking it immediately and contact your doctor.
The problems can continue even after you stop taking it because it disrupts tissue repair.
I'm off my feet right now going on two weeks. It's known for rupturing tendons.
