My first LLMD said to take it daily, not just as needed, and it would work better.

Initially I would take PRN and it worked fine for mild nerve pain. Recently I have been taking it every night to help w/ sleep.

I know...hate having to have to take anything anymore but REALLY helps with sleep.....Are you on a low dose? I am on 300 mg.'s - started at 100 mgs and worked up to 300 mg.

First prescription neuro said i could take 3 300 s a day
Usually i would take 300 during day when i had nerve pain. I think i could take 300 every 6 hrs
Sometimes i would be able to sleep with 300 but i was allowed 600 and sometimes need it
New neuro said i did not have to worry about addiction or side effects if i take less than 900 a day

I do both. That is I take it every night for pain, for sleep, and for migraine prevention. But I have 100 mg capsules and titrate the dose, according to whether I also have to take other pain meds, or if I am hyper at night, etc.
When I am not taking other pain meds, the dose is fairly constant, with some variation if I am very hyper. But when I take pain meds, I don't want to over do it with meds that could knock me out too much.
Except for now when I am still recovering from a terrible set of injuries and surgery, I was able to take opiod pain meds rarely, due to the gabapentin.
But right now it's different, due to the awful pain from the injuries and surgery. Not a good thing for a Lyme patient to have to deal with!
But my LLMD at the time said, what I already knew: the tendons were in such bad shape from the Lyme! (OT from you question).

Ty rumi...sorry you are going thru tht pain...last year was my time for two surgeries and opiods...be careful. Its easy to get dependent
