You can use over the counter cream for vaginal candida overgrowth.
However, since you have both and you may also have candida overgrowth in the GI tract, I would see the doctor for a prescription. Such as Fluconazole etc.

Agree. Some Lyme docs used to stop antibiotics and treat for yeast with Diflucan (fluconazole). It has been long enough that I do not know if they still do that or not. But that is what it sounds like you need to me.
There are herbals. But it sounds like you have a good case of it. Talk with whoever is prescribing your antibiotics.
