Because of the recent hurricanes in Florida a few months back my doctors notes and Lab results were ruined. For many months I have tried to contact her but never recieve any returned calls and I have sent her several certified letters regarding this and got no response back. She in in NJ and I really need this info as you know lyme is difficult to prove.
PLS ADVISE.

Hi Fan,
You may want to contact a state medical board and see if they have a current address. Perhaps, a state insurance department could help ? I would assume that
each state must carry some type of doctor directory ?
A Google search ?
Phone directory ? Maybe a lyme support group in Jersey can help track her down ?

I have the current address and # however her Business # is her cell phone and alls I get is her voice mail she doesnt have office staff(strange I know)....and I already sent her Cerified Letters.....GRR

You might try contacting the lab that did your tests. Maybe you would at least be able to get copies of the test results, if nothing else.

Have you sent your request certified mail, return receipt? It may not help unless you are willing or able to pursue things legally. Sometimes, though, just the sight of that return receipt is enough to convince people you are serious about what you are requesting. You would also have proof of your request if you have to pursue it further.
