My LLMD discussed a PICC with me on Monday and he said that Dr. B. is seing better results with 2g twice daily for 4 days on and 3 days off than with 2g once daily 5 days on and 2 days off.

This is why he is leaning towards 2g twice daily 4 days a week. Anyone know about this? What are you guys doing as far as dosages? Does it seem to be working at the dose you are at? I know 4g is the max dose.

Hey,

I have been on rocephin for 8 wks now @ 2gms everyday.

I kinda feel in limbo right now, so I am mentioning a cyst buster to my LLMD in the morning. I've had the Picc in for 6wks now
and that's doing pretty good.

I'm sure others will offer you more help soon, I have to run out for a little.

I infuse 2 grams a day. Three days infusing...one day off...repeat the cycle.

The only problem I would see with 2 grams twice a day is the infusion time. That would be a lot of time!

I infuse starting at dinner, and it takes an hour or longer to infuse.

To add that to my morning routine would be awkward at best.

However, I would do anything to get well!

If you're up for it, you could try what your LLMD is suggesting. If the schedule is too difficult, then go to once a day infusion.

Okay, I'm back. Well, If your going to do the 4gms did he tell you how you will be administering it? I use an I.v. push with my PICC.
It it a syringe looking thingy without the needle that screws on to the end of your extension tube and Presto! this is where the *push* comes in.
the abx I was told to take slow, Like 60 seconds per 5ml( 1/2 gm)I usually take a bit longer though. so basically less than fifteen minutes from prepping supplies to wrapping it up.. Not too shabby.

Any word on when you may be starting this?

Listen, do not go ANY faster then 25-30 minutes on your 2g or 4g IV. It CAN damage your kidneys and liver. It should take about 30 minutes.

I did 2g week one, then 4 g for two solid weeks, and then back to 2g the last week. The 4g dose DFEINATELY did more. I had a horrible herx on it that was tough, and had a constant headache and such, but it did the job. I am not seeing much results from the 2g dose.

timing for my infusion is NOT my opinion.
It states that my abx should be administered
from 3 to 5 minutes. it says it on the bag that it comes in every week. and in my paper work as well.

I'm sure your LLMD will tell you how slow or fast to go, If not ask him/her.

I've had a very bad case, but I'm doing 4 g 4days/IV push, and then the other three days I do IV zithro...I've been very ill lately, we'll give it a couple of months and see what happens...BTW, sometimes on the IV push I just do it over a minute or two...I can't see how that makes so much of a difference,2 minutes versus 5 minutes...I guess I'll slow it down now...but it's annoying.

Hi,

The 4 grams of Rocephin for 4 days and the IV Zithromax and add in plaquinel (to break up cysts) this is what I would do if I had to go on IV.

You are protecting the Liver from the Rocephin on the 3 off days and NOT allowing the Bb to HIDE in the cells by taking the IV Zithromax. Many relapse on just the IV Rocephin because it often causes the Bb to retreat into the cell for protection from the "hostle antibiotic" and then reimmerges after the IV Rocephin is finished and thus you have a relapse.

The IV Zithromax forces them out of the cell because Macrolide penetrate the cell wall. The Best of both. The plaquinel is breaking up the cysst so THERE ARE VERY FEW HIDING SPOTS.

I do 2 Gms daily for five days, then two days off. I infuse via a bag of 50cc. Following my first infusion done at the hospital (run over 20-30 minutes), I had the heart racing, pass out feeling, thought I was gonna die, vomited etc..(12 hours later) Glad I didn't have another dose in me.

I run mine over at least one hour. Way too chicken to do it any faster again. I unfortunately don't have a daily routine to have to consider yet. [Frown]

My instructions read to infuse over 5-10 minutes, but by choice and previous experiences, I prefer to run it slower. Being alone most of the time and far from the hospital, I feel if I have a reaction, I could turn it off and lessen it.

I'm also on Zith and Plaquenil. Orals.
If it were me, I would do 2 Gms once/day to start. Right now into my fourth week, I wouldn't be afraid to do the 2Gms twice a day, but think initially it may have killed me. At least that first day would have.

I spoke with my pharmacist (whose wife also has lyme so he is very compassionate and willing to talk to me).

I asked him about the infusion bags vs the IV push that is sometimes used.

His take on it was "well call me old fashioned, but I'd rather deliver a caustic drug slowly next to the heart than in a faster fashion. I don't want my patients to wind up with heart issues 10 years from now."

So...I am taking what he said to "heart" so to speak. I know that when I was infused with Rocephin at the hospital they did it over a 30 minute time period. My heart was not happy.

At home I infuse over an hour (at least) and my heart is fine.

For those of you with IV push, I would definitely push it in slower, and not faster, as per your doctor's or pharmacist's orders, for the health of your heart and veins.

Just my 2 cents...from someone who doesn't know much about this, but is learning from those who do! [Wink]
