Is there a resource for printing off a checklist kind of thing to use while going through lyme treatment to chart progress? Something like a symptom journal?
Instead of having to write out symptoms as they come and go through the different phases of treatment, and the adding or stopping of different meds and herbals, a checklist would be a lot easier.
Any ideas or resources for something like this?

You can create an excel spreadsheet with list of symptoms on left axis and days of the month on top axis.
Rate your symptoms daily with a number e.g. 0 = none, 1 = moderate, 2 = strong, 3 = severe.
Link your table to a graph that will give you a clearer picture by the end of the month.
I have a template if you need. It's a pain to fill on a daily basis, but it really helps when it comes to keeping track of the progression of the disease.

christelleny, That's a great idea. How long have you been doing this? Is it working well for you?
I'm super rusty on excel skills and at that they are only basic. But my husband enjoys using excel, so he could help me with it.
I'll have to give some thought to how to work this. A template would be very helpful as I'm not getting the picture...well, I'm just waking up, too.
What symptoms do you include? For me, my main symptoms are impaired cognitive function, fatigue, muscle aches, TMJ, headaches, weakness in legs, light sensitivity, depression, queasy stomach, frustration...
So I would list those. Do you get more exacting, like with cognitive function, spell out several aspects such as memory, focus, able to figure out bill-paying??
Then, how do you indicate when you start new meds/herbal treatments?
Any help you can give me on this I greatly appreciate.

After I thought I had Lyme but before I was dx, I started tracking my symptoms on a daily basis.
I really like the chart posted by the Lyme Disease Assn of SE Pennsylvania (toward the bottom of the page). I used the symptoms they list, but you can also get a blank chart and fill in your own symptoms.
I hit a point last week where I thought I was no longer making progress. I got out all the charts, stretched them out then easily saw just how much better I am since I started taking abx in June.
This chart was also a good thing for me to have during my first appointment with my LLMD.
