The general observation FOR LYME seem to be 4 days after startng a new drug. Exceptions to this might occur with long half life drugs and low doses (as the peak drug level will occur over time). A good example of this long half life effect is fluconozole.
If it takes longer then you might have something going on with coinfections. If it occurs earlier then it IS co infections of some sort (bartonella type stuff).
It could alo be a result of "normal" fluctuations in symtoms that occur for some unknwon reason.
Thats my take on it, for what its worth.
