Do any of you know where i can get one of these forms for my doc to complete?

I have seen some on the web for a fee, of course.

Minou, thanks so much for your prompt service to Karen with the magic links!

Karen, you want the RFC one.

Here's the key to remember when you give these to your MD to complete.

MD is to complete these based upon TODAY WHAT YOU CAN DO/CAN'T .... not the last day you physically were at work!

Also, my former lawyer had me complete a blank one over the phone with his secretary. They then sent my answers along with a BLank ONE to my PCP of 26 years & my rheumatoligist.

It was the MFC, mental function capacity, Q/A answered by the psychiatrist I went to that WON my SSDI for me!

There should be a link for that on Minou's disability links area.
