My LLMD just started me on azithromycin. She is starting me real slow, because I can have a major reaction to drugs.

I'm starting on 250 mg twice a week for two weeks, then increasing to 250 mg three times a week.

Is that insanely low? Or does it make sense to start that way? I'm also taking Omnicef.

That does seem rather low, but if you react strongly to meds, then maybe it is good to start that way. My 10yo daughter, 85lbs., takes 250mg zith daily, just for comparison's sake.

Hello Aniek.
I have been on azithromycin (Zithromax) for almost 8 months. I have been taking 500mg per day and I am 20 years old. So yes that dosage you mentioned seems quite low in my opinion, but if you are sensitive...it is best to listen to your doc's advice.

I just wanted to post this for comparison too. I herxed on Zith after a couple of weeks but then I got more stable. I just stopped the Zith and started on Levaquin (which I am not happy about...I don't reccomend Levaquin). I am also on Omnicef...600mg per day...was on 1200mg per day...but was a bit too much.

I think it's a good idea to start that slow, that's what I would do. I've all too often gotten myself in an horrible, unremmiting, constantly, escalating herx by jumping into zith too fast.

It has a 68 hour half life so builds up in your system before you know it... you're over the top.

Ok, that's how it is for me these days. It wasn't always.


Slow and steady wins the race, to my mind, and no med is any good if you can't take it. So go slow and work your way up. That is most effective for me and I'm ultra sensitive.


Disclaimer: not a medical anything at all just relating my experience and personal opinions. Surely, don't take this as advice but just as the perspective of someone that also needs to go slow to get where she needs to be.

Hi Aniek,

My LLMD started me slow also, but I never could get on a very high dosage of Zithromax. I guess I am lucky that a lower dose worked for me.

I generally took 250 mgs of Zithromax 3x a week, along with Malarone and Plaquenil.

Hang in there.

Thank you all. It's 2:16 am and I'm typing this about 6 hours after my first dose. I think slow is good for me.

I have a stabbing pain inside my joints. And there is fluid in my knees that I can hear when I bend them. I've been virtually pain free for 2 days, definitely nothing like this since I last took Flagyl about a month ago. I can't believe this herx started within hours.

I took 7.5/750 Vicodin and I can't imagine how I would feel if I hadn't. I finally gave up on lying in bed in pain and accepted I'll start my work day late tomorrow.

I believe the recommended dose for Lyme is 500-1200 mg daily. Maybe your doctor's plan is to increase it after the two weeks?

After two weeks I switch to three times a week. I think she did want to see how I would react, which was obviously strong. I was up until 4am last night because of the pain. I am really happy I don't need to take another one until Saturday [Smile]
